Ok so I got the plugin to work as far as creating a form but now when I go to profile as admin it says I have to create at least one profile form and that is exactly what I did. I'm running elgg 1.5. My prevoius issue had to do with I guess I was running it on xampp localhost but now I have it on my dedicated server.

I'm running these plugins under xampp and have not seen the problem you mention.

Consult the Form Plugin by kevin, it allows extending and creating new forms through-out Elgg.
Kevin's plugin is fantastic. However, it puts the UI in the database -- I'd rather keep the UI in the code.
Any suggestions if I want to keep things in the code using the existing groups profile approach without major modifications?

I think he gave up too! Pity really as an update to 1.7 is really desperatly needed.....
I did see ShellCode on Facebook. Apparently some people here on the Elgg community know him too, or at least communicate with him. Is it not possible to encourage him to get active here again? It was an amazing job, producing this plugin.
Site Access CATCHPA isn't working under 1.7.3 either and if you disable Site Access to use the default CATCHPA plugin then you end up with the WSOD (even after refreshing the cache etc)
Anyone come across and cured this yet?
