Description: A simple contact form that allows users to send an email via form and allows the site administrator to set address and contact information.

Features: Allows an administrator to set whether the contact form is displayed or not as well as where it is sent to.  Displays (and allows admin to change) contact name, address, email, fax, name, phone, a description and a page name.

Install: Just upload it into your mod directory.  The contact page will be at /mod/contact/ .  Remember to configure the settings!
