Admin interface to quickly edit the membership of groups

Changes are all back-end processing stuff for scalability.
This plugin adds an admin interface for editing the membership of groups. A new page menu item is added 'Groups' => 'Member Manager'
Groups are listed alphabetically showing a sample of their membership, can be deleted, or can have their membership edited. A quick link for adding a new group is also available.
Membership is managed using the elgg user picker, members can be added or removed at will (except for the group owner).
