In discussion (or other edit page), clicked embed content, then upload media.
Having browsed for a file, filled in name and description, I click upload.
There is no indication the file is uploading.
When the upload finishes, I get a prompt asking if I want to save or open the file.
I think at this point it should show the Files tab.
This was using IE 9.

Just the core 1.8 Embed plugin.
Have the same problem. After the embed, the screen gives no indication that the upload was complete, although it was. If I close the window and continue on, the embedded file is available for use. If I mistakeningly think the file was not complete and process the upload again, the file is uploaded twice.

Help me out here and I could trouble-shoot !
When i embed an image, I get only :-
<img border="0" /> ;-(
So I cannot see past that to the stage you've both described re:
".. indication the file is uploading.."
and so me not see the "problem" ;)
therefore -- unable to "debug" ;-O

@Dhrup: The problem might be a bug in tinymce - at least <img border="0" /> rings some bells. And I think I had this problem on my site (more precisely a user told me about it). The version of the tinymce editor that comes with Elgg is not the latest anymore. You might want to give my Extended TinyMCE plugin a try - either the Elgg 1.7 or the Elgg 1.8 version. It includes the latest version of tinymce (3.4.4).
ThX: funny, I do usually use TinyMCE Ext ;) almost rarely used Elgg's Embed PlugIn by itself..(btw same problem exists when I try to load images here @Comm ;-O where I usually just goto edit the HTML and put in the src= code manually which works ;-o Maybe Roger or Ron wanna give their site URL so I can test the situation there...;-0

I embed/upload re tinymce - that's where I see the problem.
This topic seems a bit old now but I am having the same problem in IE 7/8 - there is no confirmation that the File Upload has completed. Was there a resolution?
I noticed in trac the ticket #3755 mentions "was Fixed by content-type headers"? Can anyone shed more light?
