In metadata table, duplicate entries for the combination - entity_guid, name_id, value_id, value_type, owner_guid, access_id, enabled.

Some times, the time_created is also same.

I am not able to get hold of what is causing this duplicate entries to happen. Please help. Because of this, our app is getting broken.
