... Also, make sure that you're using the latest TinyMCE (from the full package or svn) and not a previous version.
On my site the same problem.
Use the full package. rc1 TinyMCE is on the bottom of the plugins list.
One file was double uploaded.

Now Have new bug if my file add Title thai language in embed Click on any file to embed it not insert to content can't show Thai Language
Thanks for this report. The embed plugin has been updated in svn with UTF-8 character support, and this will roll into the final release.
@ Ben i see on svn file media.php am Compare but not Difference
$content = htmlentities($content,null,'utf-8'); not work for me
It was playing up for me but when I moved tinymce to the bottom of the plugins list that sorted it out.
