Please note that mine is a brand new installation with only two users and only one non-bundled plugin: Themer (disabled). Obviously one difference is that you have upgraded from an older version...

Have you executed the data directory migration upgrade (and comment update)? These are long running upgrade scripts and therefore not included in the regular "Upgrade" procedure. Instead you'll see an "Upgrade" section in the admin backend. If you haven't done these upgrades yet, it would explain why the user avatars (and also comments other other uploaded files) are not available.
